When a quick return to one’s own home isn’t possible, we are dedicated to be the next best thing. Our ExceleratedCARE units offer post-acute and skilled-nursing care while acknowledging the differences in the desires and needs of each distinct patient group. We see the importance of not only returning our patients and residents to health, but keeping them there. That’s why we’ve launched TransitionalCARE, a program that assigns protocols to closely monitor the patients who are at risk of being readmitted to hospitals, and prevent that from occurring.

We also know hospitality matters, and sometimes it’s the little things that mean the most. We want to get you back to the business of regular life. But while you’re with us, we strive to offer the comforts and amenities of home – maybe even better.

My brother had been on ventilator for several weeks then sent to Bluegrass for rehab. He had a bed sore from the hospital. It got deeper and infected because he wasn’t gotten out of bed until three days before they sent him home. They told him to pee in his diaper three times then call and they would change him. Not the right way to treat another human. His clothes that we bought him disappeared along with a white box fan with his name on it. Speech therapy did a wonderful job with him and the CNAs did what they could with having no staff. He would have regressed and died had we left him there. Thank God for alert home health nurses and a loving family, he’s out of bed and getting proper care.

I want to give a big shout out to Kelly, Joe, and the entire therapy team! They helped me regain my upper body strength. Nursing staff was great as well especially Scott and Kamal.

I had a great experience at Bluegrass Care and Rehab. They treated me like family. The meals are good and the nursing staff is very nice. They made my stay here easy and comfortable.
