In order to keep our residents, their personal representatives and families, and the public informed of COVID-19 cases, the center is providing information whenever a new positive case of COVID-19 is confirmed or whenever three or more residents or staff show signs of respiratory issues within a 72-hour period. Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months. In some cases, additional training and education can be provided for staff on specialized equipment and treatment programing that may enhance recovery times. I tour with a young lady name Michelle she was very knowledgeable made me feel so comfortable and at easy . I will definitely be chosing Boulevard for moms rehab . Thank you Michelle

I'm a snowbird down from Michigan when we got here in Florida my mom got sick and needed rehab. So we chose BOULEVARD. I CAN SAY HOW PLEASE I am with the level of CARE and Professionalism we receive from the STAFF. The place is clean and well kept..THANKS TO Michelle in admission great customer service

Boulevard Rehab is a very good place to place your love ones for rehab or long term care, nursing staffs are very professional caring and knowledgeable. The physical ,speech, and occupational therapist are wonderfull. My mother was a patient there back in 2010 and received the best care i would recommend anybody to this facilitates

Boulevard is a terrible place to send your loved ones. The building and especially the rooms are extremely filthy and grotesque. When you pass by the rooms in the hallways, the smells are unbearable. The patients are not cleaned properly and it is very apparent. Most of the nurses and CNA‘s and other staff are lazy, unprofessional, and rude. They lack people skills and are in a field that requires it. You hear the patients crying for help and they are ignored. Thank goodness I visited my loved one when I did because he was not doing well, and Boulevard was doing absolutely nothing to help him. He was suffering and probably would’ve died had I not gone there when I did to send him to the hospital. I’m completely disgusted.

My Loved one stayed at Boulevard and I must say it was a great outcome and the staff was exceptional, Michelle in admissions is fantastic and the administrator Donna Lee is Awesome. They pay a lot of attention to there patients.
