Combining science with hands-on care, we provide our patients with individualized treatment plans that evolve over time to meet their needs, including psycho-social support and intensive care coordination with a wide network of partners in every community. Through our unique treatment model, CleanSlate brings increased access to people suffering from addiction or opioid and alcohol withdrawal. Drug and alcohol addiction is a chronic brain disease. We use a medically evidence-based approach to treatment, rehab and withdrawal to help our patients on their journey to recovery.

Staff is very friendly and compassionate about your problems. They don't make you feel bad for making mistakes, they just try and help you to the best of their ability. Wouldn't wanna go anywhere else for treatment.

I was Concerned about People coming in with No Masks. I had to tell the Nurse that someone was in the small waiting room Not Wearing one. I even offered him a mask but he told me he didn't need one. I told the Dr. When I went in the room to have my Virtual Visit and he seemed concerned and made a phone call after I told him. Other than that I love Clean Slate they really seem to care about every Patient. They are very understanding and I am so Blessed that We have CleanSlate in our town. I would definitely recommend CleanSlate to People needing their Care and The Whole staff is Great!! 😊❤
