Decatur Wellness & Rehabilitation Center, formerly known as Grace Healthcare of Decatur is undergoing some exciting changes. We are a skilled nursing facility located off River Road, our goal is to provide the best nursing care in the counties of Meigs and McMinn.  We focus on implementing continuous quality improvement measures across the entire spectrum of the healthcare services we provide.  We will continue to seek better standards to provide a loving, welcoming and encouraging environment so residents can reach their highest potential in physical, emotional and social well-being.

Amazing life enrichment program! The entire staffing team in this facility goes above and beyond to assist these residents with everything they need! They have fun! Their voices are heard! Their activities program is all based on things they enjoy doing and they choose the things they enjoy doing the most! Example! Recently some of them gathered and hand made this painting to send to Florida! To a very special friend of our facility! It’s a small town friendly facility that makes everyone feel welcomed and at home! The administration in this facility is top notch! This team is led by an administrator who is educated in multiple fields of long term care! And has the residents well being as her main concern! Decatur wellness & Rehab center! Is the best! My dad was there for rehab! And I could not have asked for a better TEAM of people to care for him! I totally recommend this facility and this TEAM

The people staying there don’t get the proper care they deserve. Takes nurses awhile to pass their meds to them. Takes nurses assistants awhile to answer their lights and care for them. A lot of the staff is not nice. They are short staffed. They are hateful and have attitudes. You never see people in the office out on the floor helping out when it’s needed. Have notice before and heard from family there’s no communication in the building. Just a bad place all around.Had a family member there and they were not happy at all. They need a new person running the building and better people working there.
