Subacute Care Subacute care is comprehensive inpatient care designed for someone who has an acute illness, injury, or exacerbation of a disease process. It is goal oriented treatment rendered immediately after, or instead of, acute hospitalization to treat one or more specific active complex medical conditions. An interdisciplinary approach is used to formulate a goal-oriented plan of care for each resident. Longterm CareLong term care (LTC) is a variety of services which help meet both the medical and non-medical need of people with a chronic illness or disability who cannot care for themselves for long periods of time. It is common for long-term care to provide custodial and non-skilled care, such as assisting with normal daily tasks like dressing, bathing, and using the bathroom. Respite Care HMH Prospect Heights Care Center offers short term care services for individuals in need of 24-hour nursing care for short periods of time. Respite Care is typically beneficial to caregivers who are taking a vacation, are on a business trip, or who simply need a caregiving break. Respite Care can be for as little as a few days and as long as a few months. Rehabilitation Services Our fully licensed Physical Therapy staff works with each resident to develop a personalized treatment plan that will restore, maintain and promote function, strength, endurance, balance, and mobility, while preventing or limiting the permanent physical disabilities that could occur. Our staff works in concert to insure that the best possible therapy plan is followed at every step of the patient’s recovery.

My father had his third successful stint at Prospect Heights. The facility is immaculate, the nursing staff is top notch, the aides treated my father as if he were their father, and the therapies got my father rehabilitated and back home! I cannot say enough good things about our whole Prospect Heights experience. After my father was discharged, I had some follow up questions and concerns, which were addressed by the staff immediately. I hope to not have to use Prospect Heights again, but if I need a rehabilitation facility, I will look no further than Prospect Heights!

I will post with details at another time. For now, just want the public to know that under no circumstances take your loved ones to this place. I’ll give it one star because of only one person that did do his job and was very helpful. The rest of the staff, neglected my uncle’s health and because of this facility, he passed yesterday. Details to follow.

I am flabbergasted. Before I wrote this review I skimmed through others. I very rarely write reviews. The experience we had was what we would have hoped for for our 82 year old mother. Staff was very attentive, caring, communicative. We never felt like a worry having her there. When it came to discharge, Jane and Alisa made the process very smooth and held our hands through the process.

This place was a very big disappointment. My aunt went here for physical therapy after a major operation. She was put in a room with no hot water and was under the impression there was no hot water anywhere in the facility. But after questioning it several times; she found out it was only in her room. Even though there were other rooms available WITH hot water; they didn’t move her until almost two weeks after she was admitted. How unsanitary!!!
