We’ll review any previous medical records or radiology results, and evaluate your overall health and fitness. Once your needs and goals have been established, you and your team will work together to design an appropriate treatment plan.

We’ll keep you informed along the way—answering any questions or concerns—and make sure you are prepared for every appointment and beyond.

How to schedule an appointment

A referral from your provider may be needed before your visit. You may call the IU Health Physical Therapy & Rehabilitation central scheduling phone number at 800.248.1199 or contact us directly at 317.688.5990. Please provide a 48 hour notification to cancel or reschedule an appointment.

Free surface parking is available, including 8 designated spots and additional handicap parking. We are located in Merchant's Square at 116th Street and Keystone Parkway. Our location is behind MCL and across from World Market and Planet Fitness.
