Your hospital physician communicates medication and treatment orders at the time of your hospital discharge. These orders will be implemented to help provide continuity of care.
Upon admission, the interdisciplinary team will work with you and your family to identify care needs, preferences, and goals for your stay. You are a valuable partner in the process.
Within the first few days of your stay, you and your family will be invited to participate in a care planning conference to discuss needs, discharge planning and progress toward goals.

The Last One1 StarThis place needs to be shut down. Don't bring your loved ones here to be cared for because I can tell you that they will not be looked after in the slightest. The CNA's don't care and manhandle residents. If there is an accident with a resident and 911 needs to be called- it wont get done. No one will answer any incoming calls at the front desk. The receptionist will put you on hold while she jokes around with other staff. The staff in general are extremely incompetent and I truly pray for those that come here seeking care and rehab. This place is the embodiment of everything wrong with the world.Wednesday 25th August 2021

LeVell Griffin1 StarI was a patient in in this facility for exactly one week I had to get the hell out when I enter the facility I was treated so horribly I was thrown in a back room and forgot about I arrived on a Friday did not receive a meal until Sunday had to order Uber Eats did not have a television until Monday please do not send your loved ones to this facility I requested every day to see administrator and I was always told the administrator was in a meeting I was very ill but I choose to go home because if I would have stayed in that place I would be dead right now I know that they are going to read my comment and say thank you for bringing this to our attention but nothing will be done I promise you get out the businessTuesday 14th December 2021
Tiffany Ray5 StarThis is an awesome place to work. The management team is a true team. Andy, the administrator really promotes team work, and looks for the right person to fill his management positions versus just hiring a warm body. LaSandra, one of the social workers is amazing! Tonjua and Isiack are the unit managers and are two of the best. The are quick to respond to concerns from residents and families. Joreme, the ADON, offers support to the unit managers and is very passionate. If you are looking for a great place to grow in skilled nursing and be part of a team, this is a great place. I'll definitely miss this team, and wish them well in their search for a DON to complete their team.Sunday 1st November 2020
