This place is just awful! They don't care about the residents and lie to cover each other. I have personally seen them ignore call lights for more than a half hour and leave the door open while doing personally care to residents. They don't bath the residents regularly unless they can transfer without a hoyer. The agency temp CNA'S and nurse's don't come back more than once because this place is so horrible. I've heard nursing staff say a resident can go to the bathroom in their pants because they have a brief on when the residents asking to use the toilet.

My grandfather was here for a short time but a few people left a lasting impression good and bad. Amelia in social services Social Services was always so helpful with answering our questions and returning our calls. She worked very hard with the housekeeping team to find my grandfather's clothes when they came up missing. She even put his name on his clothes so it wouldn't happen again. The therapy department did a great job getting him back to where he needed to be. Unfortunately I can't say the same about the nursing team. They were not very helpful with returning calls when we left messages nor did they really answer the questions we had.. most of the time they were short and rude with my wife and I. The food here wasn't the best either, my grandpa used to complain about it all the time.

It’s taken me almost 6 months to share my review of this horrid place due to grieving. My grandma was taken here for what was supposed to be rehab after COVID last august. When she went into the hospital she was walking on her own, bathing herself, cooking her own food etc. After a month of back and forth between Baptist south hospital and Orange park consulate, her false teeth were lost, she could no longer walk because they had never gotten her out of bed, was laying in her own feces with a bedsore from her tailbone to middle back that was oozing and wide open!!! I literally almost puked when they finally let me in to see her. She suffered so much under their “care”. Her room was filthy, she’d lost 30 pounds at least which tells me they weren’t making sure she ate anything. They blamed COVID protocol for not letting us come see her and we called every single day to try to talk to her but they literally would tell us “they couldn’t find her” when we called. It was unbearable and incredibly heartbreaking to be so helpless. The day I was finally let in was the day she passed and I was with her for 8 hours literally being her nurse and not one time did anyone come to check on her while I was there. This place is making money from COVID patients and literally letting them die. So so wrong…would NEVER send anyone to this place.

My aunt was here for a short time for rehab. Her 2 biggest complaints about this place was the lazy night shift and the food. She sent me pics of the food a few times and it looked horrible! She would tell me about the night shift staff not answering her call light or they would come turn it off and ignore her. 😒 She loved the social worker and therapy team. She told my mom and I about how Emilia made sure she knew how important it was to follow up with her Dr and to keep doing therapy once she's home. She helped arrange things for my aunts discharge which made her feel good about going home.my aunt told us how motivated the therapy team was and they made sure she was always up and going! Thank you for helping my aunt come home!

First off, the rehab personnel are great. Some of the nursing staff is great. The CNAs are different story. I was in the center for 5 weeks. My packet from the hospital was put behind the desk and not even looked at the medical packet from hospital. I know this because I had a follow-up appointment with a surgeon 4 days later. They had to call and reschedule my appointment because they never set transportation. I had to wait two additional weeks. Now I was 3 weeks post amputation surgery when I finally got to my first dressing change appointment. The doctor left specific instructions for wound care which was supposed to start that night. I had to ask five times about getting the wound care the doctor had ordered before someone did it. The nurse in charge at the desk did not put the order in my folder or into the computer for them to see. Another nurse found the paper on the desk at the nurses station after one of my family members had to talk to the DON. The next day, wound care was not done either. I had to ask nine times for help. The 10th time I asked, it was finally done, 7 and 1/2 hours late. Being non weight-bearing you have to use a bedpan or have a potty chair that you can slide onto. They do not have one that would accommodate my larger size. Because of this I had to use a bedpan. They would rather stick a diaper on someone who can use the toilet then take care of them. I would have to use the bathroom and push the call button and wait hours to use the bathroom and then hours to get back off the bed pan. It is painful both ways. In the 5 weeks I was there, I only received one bath. If there is no way for a patient to walk four-wheel themselves to a shower room, you are not getting bathed at all. I at feeding time, the selections are atrocious most of the time. Portions are very small, like what you would feed a toddler. 7 days a week there is oatmeal toast and scrambled eggs. There is no offering of anything to put in said oatmeal to make it taste better and all hot food is served room temperature. I have my food manager search notification and the food is definitely not being served hot enough. Patients don't get the milk that is put on their food tickets every day. You only get milk if you call ahead and ask for a cold cereal instead of the eggs and oatmeal that is served. But there's no way to call the kitchen. Less than 2 weeks after being discharged from this facility, I am back in the hospital and this time I have mrsa. I thoroughly believe I caught it at this facility due to their negligence in wound care. I do not recommend anyone take their family member there unless they don't care about them.
