To make an appointment, please call our office at (757) 234-4285. If you need to change your appointment time, we appreciate notification at least 24 hours in advance of your scheduled visit.

Please arrive 15 minutes early for your first appointment so that we may get to know you and obtain important health information.

What to bring to your first appointment

If you have been or are currently being treated by a physician of Riverside Medical Group, we can access your medical records electronically. If not, and if at all possible, please bring copies of relevant medical records.

Updated: After emailing Warwick Forest, they quickly took care of the issue with his missing items. Additionally, my friend needed medical attention and was sent back to the hospital. His daughter, who lives with me, did not receive a call as to his condition or him having a medical issue. I later found out that Warwick Forest did try to call but had the wrong number on the phone contact. I was going to get him in another facility but he stated that the staff and food at Warwick Forest was very good and he would rather go back there. Once they did get in contact with us, they were very nice and apologetic over the issues. They explained everything that had happened while my friend was there and both I and his daughter were satisfied with the information they provided. I agreed that he should go back there once I found out that they were indeed treating him very well and giving him the care he needed. Original Review: A friend of mine was admitted to Warwick Forest Rehabilitation Center by Riverside Regional Medical Center. His disabled daughter lives with me and I have been handling his affairs while he was in the hospital. He asked me to bring him fresh clothes, shoes, razor and other personal items to the hospital before they transferred him. The hospital nurse told me he was already transferred and I could take his items to Warwick Forest. I called and was instructed to place his name and room number on the bag. I did exactly what they told me and my friend has called me several times because no one can locate his items.
