When a quick return to one’s own home isn’t possible, we are dedicated to be the next best thing. Our ExceleratedCARE units offer post-acute and skilled-nursing care while acknowledging the differences in the desires and needs of each distinct patient group. We see the importance of not only returning our patients and residents to health, but keeping them there. That’s why we’ve launched TransitionalCARE, a program that assigns protocols to closely monitor the patients who are at risk of being readmitted to hospitals, and prevent that from occurring.

We also know hospitality matters, and sometimes it’s the little things that mean the most. We want to get you back to the business of regular life. But while you’re with us, we strive to offer the comforts and amenities of home – maybe even better.

I initially started with the fear I wouldn’t fully enjoy it, thank god I was wrong. I’ve never felt so at home and relaxed whilst on the clock. I was honestly shocked when I wasn’t dreading going in, but actually looking forward to it. Everyone supports each other and go out of their ways for not only other staff, but also the residents. Whether it be staying after even on a long shift, or still wrapping up when at home to ensure the best outcome for all. I can see myself creating a career and being happy

This is a for-profit business. I hope you will invest in some bariatric bed sheets Or even regular bed sheets, so people don't have to be laying on mattresses. As a visitor I was appalled that some of the residents don't have sheets to sleep on. Also, some of the aides are walking around with earbuds on, you can't catch their attention. I think you ought to have that for after hours or lunch break only. This place needs to tighten up.

Came to Jefferson Manor with high expectations after working in rehab facilities through out Louisville. I have been very pleased with the facility and how it treats its residents and staff. Looking forward to a long career at Jefferson Manor.

My stay at Jefferson Manor was wonderful. Everyone was great to me. All shifts did a good job. Everyone checked up on me timely. The third shift staffed seemed exceptionally kind and polite. The facility was clean and the food was good. I haven't ran into anyone I can't get along with.
