When a quick return to one’s own home isn’t possible, we are dedicated to be the next best thing. Our ExceleratedCARE units offer post-acute and skilled-nursing care while acknowledging the differences in the desires and needs of each distinct patient group. We see the importance of not only returning our patients and residents to health, but keeping them there. That’s why we’ve launched TransitionalCARE, a program that assigns protocols to closely monitor the patients who are at risk of being readmitted to hospitals, and prevent that from occurring.

We also know hospitality matters, and sometimes it’s the little things that mean the most. We want to get you back to the business of regular life. But while you’re with us, we strive to offer the comforts and amenities of home – maybe even better.

Since becoming a part of the Tanbark community, she has transformed into a more successful and happier person. This shows in her new found zest for her job which I have seen in her after work attitude. Less stress and much happier in her after work pursuits. Good job Latoya Morton. I stayed at Tanbark Rehab for 4 weeks to rehab my broken leg. The facility was very clean and comfortable and the food was very good, too. There were numerous people to help with anything you needed. and physical therapy.

For me, it was a refreshing moment each day to see my room ambassador. She made a difference each day I was there. Nurses are a professional group led by Justin. Several specific ones are outstanding. The Therapy team are a truly crackerjack team. Noted for their skill, care, and patience. I highly recommend Tanbark for any Rehab needs.
